Telephone installation, re-installation and re-execution of the agreement on telephone use

1.2	Change of telephone number by subscriber’s request	40.00
2	Re-installation (transfer) of telephone connected to a separate line with retaining the phone number
2.1	when a subscriber changes apartment within one house	40.00
2.2	when a subscriber re-locates house within one ATE	60.00
3	Re-registration of the agreement on telephone use
3.1	when a subscriber moves for residence to another premises where telephones were installed by Ukrtelecom JSC	7.50
3.2	in favor of a family member or other co-owner of the apartment	7.50
3.3	in favor of a new owner of premises under written consent of the subscriber-owner of option (R-class,  F-series)	7.50

Notes:

*200 included minutes of local communication shall be counted against subscriber fee.
